POSITIVE BEHAVIOURAL SUPPORT

At Caretms, we use the positive behavioural support (PBS) framework to assess and create a behavioural support plan (BSP) that is person centred and meet our service users changing needs and support behaviours that challenge

Our service ensures our service users are at the heart of their support planning and delivery. Our service users are in control of their care and support and reserve the right to amend such as they wish

Our service works closely with service users, families, carers and legal representatives to shape our service delivery tailored to the service users needs and agreed outcome

We conduct a robust joint positive Behaviour Support Plan (BSP) that maintains accountability and protects service users from harm

We provide a service of “preventative care” because we believe in preventing our service users from harm and abuse before it happens. However, where abuse or harm cannot be prevented we have a positive action plan in place to protect our service users best interest

We conduct Functional Assessment to identify and mitigate against behaviours that challenge

I can say what matters to me such as where to live, who and how to support me. Therefore, I experience exceptional quality care and support from my staff. I trust my carers because everything related to my care and support, I am involved in each decision-making, and I get help to change things I don’t like. I am the most important person at meetings that are about me and that helps me to live a fantastic life.

Our organisation provides the right care at the right time to ensure a high level of care for our service users. We take time to build trusting, supportive, empathic, and non-judgmental relationships as an essential part of care. Our service users are in control of their care and treatment. Every car plan is individualised to each service user’s needs and wants.
Our staff are aware of each individual's needs and work closely with service users to ensure their care plan is maintained and they receive the right to amend at any point of care: before, during and after care and support with consent.

We design the care plan based on individuals bearing in mind their dignity and respect. We listen and encourage family and advocates to bring their contribution to develop care plans jointly to enhance the service users’ quality of life 

My family and my staff get to know each other by assisting in training to keep me safe and well when visiting family

We work in partnership with our service users and their family, when providing training to staff, family and friends are invited to participate in our tailored training session to improve the quality and experience of care of our service users. 

Having the right to choose who I live with is important as it makes me feel like I want to be at home.

We make sure that the service users control what happens in their home and that their rights and privacy are respected. The culture of our home is that everything should be driven by the service user's wishes and feelings, enabling them to enjoy life and live in the moment. 

I am so happy; I have regular health checks. My staff always remind me when I have GP or hospital appointments. I also have an updated optician, dental & hospital passport with my picture, my contact details on it and all the information regarding my medications and how to look after me.

It is essential that annual health checks continue to be carried out to improve our service users’ health by spotting problems earlier. We work with partner organisations to help our service users to live healthier and happier lives. 

I love my community, I am supported and encouraged to explore every avenue to choose the right job for me where I can share my talents and skills with my neighbours. I am also inspired to learn new skills by getting a different job. I discovered that work experience is one of the best ways to get experience and to meet new people. Another great way to gain experience is through volunteerism.

We believe in social inclusion and recommend our service users to be part of the community activities, hobbies, and interests that we see they might like. We promote and maintain their independent and self-efficacy, wherever possible. We ensure that these commitments are individualised and fit around their care plan. Diversity allows our service users to solve complex problems creatively. Divers’ experiences are so valuable for our service users, it helps them to become more creative and embrace life with positivity and happiness 

I feel comfortable, productive, and safe because my staff are prepared and prevent accidents from happening by using different techniques to always protect me.
Service users and staff safety are at the centre of our care provision. We provide staff with tailored training on how to reduce the level of conflict in crisis situations. Our staff know how to build rapid rapport and a sense of connectedness with the service users to reduce the challenging behaviours. Thus, increasing the level of safety for everyone involved 

Our service is safe and responsive, and our continued duty of care goes beyond our environment. With this in mind, we ensure staff always accompany our service users when hospitalisation is required to ensure service users are always comfortable and someone they recognise is always around in a time of crisis.
We believe this will put our service users in an atmosphere of hope and optimism, with a clear focus on their emotional, psychological needs and preferences. And we ensure that they are look after without discrimination or fear and do not stay beyond medical necessity.
